Cleft orthognathic surgery is a specialized procedure performed by oral and maxillofacial surgeons to correct jaw alignment and improve facial structure and function in individuals with cleft lip and palate (CLP). It's an essential part of a comprehensive cleft care plan, aiming to address skeletal and dentofacial deformities, and improve facial symmetry, occlusion, and airway function.
What it is:
Orthognathic surgery involves repositioning the maxilla (upper jaw) and/or mandible (lower jaw) to correct malocclusions and improve facial aesthetics.
Why it's needed:
Cleft lip and palate patients often have a 3-dimensionally deficient maxilla, leading to a class III dentoskeletal malocclusion and other functional and aesthetic issues.
Common procedures:
Maxillary advancement: To address midface retrusion.
Mandibular setback: To address relative mandibular protrusion.
Other procedures: May include alveolar bone grafting to augment deficient bone.
Timing:
Orthognathic surgery is typically performed during late adolescence or early adulthood, usually between the ages of 16-22 years old.
Benefits:
Improved facial symmetry and aesthetics.
Better oral function, including biting, chewing, and speech.
Improved airway function.
Enhanced quality of life.
Challenges:
Residual sequelae of multiple previous surgeries.
Dental differences and unhealed fistulas.
Scar tissue from previous surgeries.
Tissue deficiencies.
Difficulties in aligning multiple segments of bone and soft tissues.
Multidisciplinary Approach:
Cleft orthognathic surgery is best approached with a multidisciplinary team, including surgeons, orthodontists, and other specialists.

Live surgeries covered were LeFort 1, Distraction Osteogenesis with RED distractor and Bi Jaw surgeries.
